WHAT IS THE BEST WAY FOR US TO APPROACH YOUR PET?(Required)
We try to be as stress-free as possible for both you and your pets, so if you know something that your pet loves (treats?) or hates (touching the feet, doorbell, etc.) please let us know.
We want to make our visit as stress free for you and your pet as possible. By creating a low-anxiety, positive experience for our patients, we can help decrease the fear, anxiety, and stress of veterinary visits over time. We do not blame pets for being scared of the veterinarian as we often reinforce their fear by doing things that they do not experience in their typical day-to-day-lives (restraining them, touching parts of their body they are not used to, and often performing mildly uncomfortable procedures like blood sampling or vaccinations).
We want to work as a team to try and decrease the stress of appointments over time, but we also need the following basic requirements to be able to examine your pet:
-You should be able to open your pet’s mouth and touch your pet's feet and ears.
-Your pet must be able to be held still and touched by a stranger for at least 2 minutes so an examination can be performed.
-If your pet shows signs of aggression - snarling, biting, growling, hair standing up on their backs, etc., then we will take safety measures such as applying a muzzle for the duration of their examination to protect you, our staff, and to prevent your pet from being quarantined due to a bite occurring.
If your pet cannot meet the above criterion, they may need behavior medications to decrease their fear, anxiety, and stress to allow examination. Many of our patients receive anti-anxiety pre-medication for their visits and do very well. By decreasing their anxiety, we attempt to counter-condition them to accept veterinary visits as they learn it is not as scary as they once thought. We hold ourselves to a high standard and want to provide the best medical care to your pets, but if we cannot touch or listen to them, we cannot examine them thoroughly.
In some cases, animals can be so scared or painful that they may require full sedation for examination or a procedure.
You, the undersigned owner, are not allowed to hold your pet. If you insist on holding, and you are scratched or bitten, you release the veterinarians at Symphony Mobile Veterinary Clinic, Symphony Mobile Veterinary Clinic PLLC, their agents, staff, servants and representatives from any and all liability.
Please be aware that if we are unable to fully examine your pet due to behavior concerns, we still charge the full house call fee. We will make all reasonable and safe efforts to examine your pet. We want this to get better over time and not make the animal more fearful in the long term. We need to work together to minimize fear, anxiety, and stress. We love animals and we want them to love us too!

*Due to the nature of mobile practice, we have different resources related to pet restraint and patient care versus traditional hospitals. We are not the right fit for every family or every pet. If we see anything that indicates that our standard of care or staff safety will be compromised we will cancel your appointment.
*Please know that we cannot provide emergency care, hospitalization, some soft tissue and most orthopedic surgeries.
Payment is expected at time of service. We accept cash, all major credit cards, debit cards, checks with a valid ID, and most online payment platforms. Returned checks are subject to return check fees.
Please understand that there will be no refunds or exchanges of any kind. If payment is not received at the end of the visit, payment is canceled, card is declined, card charges are disputed, or if a check is returned you will be charged a $500 late/no payment fee that will be added to my current bill.
*If you or your pet is 15 minutes or more late to your scheduled appointment or agreed upon time you will be charged a $75 cancelation fee. Once the fee has been paid, we will allow scheduling for another appointment.
*If you cancel your appointment within 24 hours of your scheduled time, you will be charged a $75 cancelation fee.
*Please know that traveling in excess of 20 miles from PetSmart in Goldsboro will result in a $7/mile travel fee with a maximum of 70 miles.
*Symphony Mobile Veterinary Clinic has zero tolerance for aggressive behavior and threatening language. SMVC is committed to providing a safe environment for patients, clients, visitors and staff, and we reserve the right to refuse our services and/or cancel the appointment at any time.
* I, the undersigned owner or authorized agent, release the veterinarians at Symphony Mobile Veterinary Clinic, Symphony Mobile Veterinary Clinic PLLC, their agents, staff, servants and representatives from any and all liability to the loss or damage of property and persons, including any and all injuries to pets, self, or others, whether it be emotional, physical, psychological, accidental, intentional, distress, financial, reputational, etc., that may be sustained by me, or any of the property belonging to me, or others whether caused by the negligence of the releasees, or otherwise, while participating in such activity, or while in, on, or upon the premises where the activity is being conducted by Symphony Mobile Veterinary Clinic, Symphony Mobile Veterinary Clinic PLLC, their agents, staff, servants and representatives.
I, the undersigned owner or authorized agent, voluntarily and with full understanding, consent to the physical examination of my pet. I understand that the procedure may involve things that make my pet feel uncomfortable (ie. vaccinations, restraint, fecal samples, etc.) at times, but is for the benefit of my pet.
* I, the undersigned owner or authorized agent, agree to the use of pictures and video to be used of my pet for continued educational purposes, social media, and for personal use of Symphony Mobile Veterinary Clinic, Symphony Mobile Veterinary Clinic PLLC, their agents, their social media accounts, their emails, staff, servants and representatives.
